Once you login and navigate to the form. It's easy to complete as an electronic form is available to populate with information, attach applicable documents and submit directly to the CFO of Smiles® team.

The information is stored in a software that follows ISO/IEC 27001:2022 specification for an information security management system (ISMS). To ensure the system is working, there is a Service Organization Controls Audit (SOC 2 Type 2) that is completed to assess the protection of practice and patient confidentiality.
